Why is Gynecomastia Surgery Needed?

Gynecomastia is physiological and does not usually require surgery when it occurs during puberty or in older age. This surgery is performed under general anesthesia (the patient is asleep). A small incision may be made around the areola or under the breast crease, depending on the size and composition of the breast tissue (fat or glandular tissue). Gynecomastia is classified into three degrees. In the first degree, the breast is enlarged. In the second degree, the breast is enlarged with some excess skin. In the third degree, both the breast and skin are significantly enlarged. For patients who are not in the third degree, I suggest removing the excess breast tissue and allowing the skin some time to retract before considering skin excision, as scarring, whether it heals well or not, will be permanent.
Techniques Used
The surgery can involve liposuction, power-assisted liposuction, laser treatment, and/or direct excision of the breast tissue. When liposuction is used, a small incision is made under the breast fold. If the breast tissue is surgically removed, an incision around the areola may be used. These two procedures can be done separately or in combination. In advanced cases of gynecomastia where there is significant excess skin, the skin may be excised, and the nipple repositioned or transferred to a different location.
